Sometimes — but not a decision you make from a product page. U.S. electrical code has provisions for replacing a fixture at an outlet that never had a grounding conductor, and the edition in force plus local amendments vary by jurisdiction.
Knob-and-tube adds a problem most guides skip: its insulation is rated for roughly 50–60°C, while modern fixture terminals expect wire rated to 90°C.

You bought a heavy metal chandelier for a 1920s dining room. The old fixture comes down, and there are two wires where you expected three. This is one of the most common ways an old-house lighting project stalls.

Three conditions turn up in old ceilings. They are not variations of one thing, and they lead to different costs.
Why “no ground” is not automatically a no
Electricians meet this constantly, because millions of American houses were wired before grounding at lighting outlets was normal. The code recognizes the situation and sets conditions under which a replacement fixture may go in where no grounding conductor exists. What it does not give you is a blanket yes: the edition your city adopted, the amendments on top of it and the protection on the circuit all matter, and none are visible from where you stand.
There is a shopping-side answer that sidesteps the issue. A fixture with no exposed metal — fully insulated body and canopy — does not raise the same question. Ask whether that changes your electrician’s recommendation before assuming the ceiling has to be opened.
The knob-and-tube heat problem
This is why electricians often will not connect a new fixture to knob-and-tube even where the wiring looks intact. Fixture terminals are designed around conductors rated to 90°C; knob-and-tube conductors are rated far lower, commonly 50–60°C, and fixtures run hotter inside the canopy than people expect.
The usual route is to terminate the old wiring in an accessible junction above the ceiling and bring modern cable down to a proper fixture box. That is real work, priced accordingly, and worth knowing early.

- Is any wiring feeding this room knob-and-tube, and how much of it?
- Under the code edition in force here, what are my options at this outlet?
- Would a fixture with no exposed metal change your recommendation?
